Guess you didn't get any Indian promos for extras. I got the back rest, sissy bar and passenger boards. I like the adjustable back rest and it's easy for me as a short rider to swing over the seat. You may want to consider Indian OEM

Anyone know of any aftermarket driver backrest for the Indian chief vintage?

Dave
Grasshopper, Ltd. in Beloit, Wisconsin. Makes a real nice driver backrest for less than half the cost of the Indian backrest. Check their web site for details. Had them on my Victory's and now my Chieftain. Good quality and easy to install.

Is this only the backrest pad or does this come with the mount, stem (that inserts to the seat), and the backrest pad?

I have the Kuryakyn. Easy install and works well. I only use it for cross-country rides though since I don't care for the look of a backrest on a one-up seat otherwise. But it's great around the five-hour point, looks be damned.
